Case Manager – Social Worker

Responsibilities

• Coordinate and provide care to clients that are safe, timely, effective, efficient, equitable, and client-centered.
• Provide case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post‑adoption support for birth parents.
• Create a service plan in conjunction with the client to develop goals and objectives that are consistent with what the client wishes to accomplish.
• Counsel, advocate, and teach clients the skills identified in their service plan face‑to‑face in home or virtually.
• Respond to client’s crisis situations within program parameters in a timely manner according to program protocols.

• Bachelor's or Master's Degree in Social Work, Counseling, Psychology, Criminal Justice, or other Human Behavior Science.
• Candidate must be a licensed driver with access to dependable personal transportation and auto insurance.
• Passion for making a difference and supporting families through life’s challenges.
• Preferred: 2 years experience with Child Welfare, Adoption, Foster Care, Community Resources or Care Coordinator.

Demonstrates expertise in case management, client advocacy, and crisis intervention while providing client‑centered care. Proficient in developing service plans and delivering education and support in areas such as pregnancy, parenting, and adoption.
