The Prevention Program is funded through the Administration for Children's Services provides comprehensive home and community-based preventive services to children and their families. The Program integrates Mobility Mentoring, an evidence-based model that is grounded in science emphasizing a growth mindset, high expectations and belief and investment in all children and families. It serves as a collaborative team member using the six-stage Family Team Conference (FTC) model and to support all families during their participation in the Prevention Program.

The FTC Facilitator serves as an integral member of the Prevention team ensuring family voice and choice as well as the core principles and practices of Mobility Mentoring are integrated into the work and planning with all families. Mobility Mentoring, an evidence-based model is grounded in science emphasizing a growth mindset, high expectations and belief and investment in all children and families.

The FTC Facilitator works to support and ensure the stability and growth of families based on their identified goals and plans to ensure children and families have access to and are linked with resources and services that help them problem solve and support families in reaching their optimum goals and achievements.

The Facilitator coordinates and performs an array of family team conferences (FTCs) involving families enrolled in the Prevention Program. The Facilitator is responsible for using the Administration for Children's Services (ACS) approved six-stage model for ensuring that every family's voice and choice are included and highlighted during conferences that address the prevention services in place or needed, as well as the family's progress towards achieving outcomes.

The FTC Facilitator is also responsible for fully understanding and supporting the integration of Mobility Mentoring into each family's plans and goals and working collaboratively with the Prevention Program team to support the achievement of goals.

Responsibilities:

Facilitate FTCs for families and staff to review services, assess progress together, and help develop a plan of action to either ensure further progress towards positive child and family outcomes or end services as appropriate

Utilize the six-stage model for FTCs and encourage and support the use of Mobility Mentoring in interactions and planning with families and staff

Maintain an active and flexible FTC schedule that meets the needs of families and compliance guidelines of ACS, including responding to crisis intervention, changing family needs and/or newly required FTCsServe as an active member of the interdisciplinary team including attending case conferences, reviewing each family's case record in preparation for the FTC and maintaining communication with team members during the life of the case to support the integration of Mobility Mentoring and positive family outcomes

Conduct visits to participant homes in order to hold conferences when needed

Submit accurate and timely documentation summarizing each FTC as required by ACS in PROMIS and Connections databases
