A skilled nursing facility in Brooklyn, NY is looking for an experienced Staff Social Worker for their sub-acute unit.
Salary: $75K to $90K range (based on experience)
Qualifications:- Must have BSW or MSW
- Must have 2+ years experience as a Social Worker in long term care
- Must have experience in a nursing home setting
- Must have strong organizational and case management abilities
- Excellent communication and interpersonal skills
Job Description:
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
