West Side Federation for Senior and Supportive Housing (WSFSSH) seeks a Bilingual Social Worker II to support low-income older adults and people with special needs living in affordable, supportive housing. In this field-based role, you will provide bilingual case management, counseling, and crisis intervention; complete biopsychosocial assessments; and coordinate services that promote independence, housing stability, and wellness. You will advocate for access to benefits and healthcare, collaborate with an interdisciplinary team, engage families and community partners, and maintain detailed documentation, all within a mission-driven, resident-centered, and social justice–focused environment.
Key Responsibilities- Provide bilingual case management and supportive counseling to low-income older adults and individuals with special needs in WSFSSH housing
- Conduct biopsychosocial assessments, develop person-centered care plans, and coordinate appropriate services and referrals
- Advocate for residents’ access to healthcare, benefits, and community resources to support independent living
- Collaborate with property, nursing, and program teams to address resident needs and promote housing stability
- Maintain accurate, timely documentation and comply with agency, funder, and regulatory requirements
- Facilitate groups, educational workshops, and crisis interventions as needed
- Engage families, caregivers, and external providers to support resident well-being
- Promote a respectful, trauma-informed, and culturally responsive environment aligned with WSFSSH values

- Training in trauma-informed and person-centered care a plus
