TAG Med Staffing is hiring an LMSW Coordinator (Clinical Outreach) in New York, NY for our client, supporting case management operations, clinical supervision, and housing placement initiatives within a community-based services environment. This role focuses on overseeing case managers, ensuring high-quality service delivery, and implementing best-practice interventions to support individuals transitioning to stable housing. The position offers the opportunity to provide leadership in a fast‑paced, mission‑driven setting.
Responsibilities- Team Supervision: Manage and support a team of Case Managers to ensure effective service delivery and performance.
- Clinical Oversight: Provide clinical guidance and direction to support best-practice interventions and client engagement.
- Case Management: Oversee development and execution of individual service plans and ensure accurate documentation.
- Client Assessment: Conduct assessments and evaluate client needs to support appropriate service planning.
- Crisis Intervention: Support staff with risk assessments, incident management, and de‑escalation strategies.
- Care Coordination: Collaborate with outreach teams and service providers to support housing placement and continuity of care.
- Education: Master’s degree in Social Work or a related field is required.
- Licensure: Active New York State LMSW license is required.
- Experience: Minimum of 3 years of experience working with similar populations is required, with supervisory experience preferred.
- Industry Knowledge: Strong understanding of Housing First, Harm Reduction, and trauma‑informed care models is required.
- Technical
Skills:
Proficiency in Microsoft Office and ability to manage documentation systems is required. - Skills & Attributes: Strong leadership, communication, and problem‑solving skills with the ability to manage multiple priorities are required.
