Location: New York
NYU Langone Health’s Family Health Centers is seeking a Counselor in Brooklyn, Monday–Friday, 9a–5p, with Spanish or Mandarin speaking required. The counselor will screen for social determinants of health, assess eligibility for the 1115 Medicaid waiver, assist with benefit applications, and advocate for clients to access necessary services.
Responsibilities include outreach, referrals to public and community agencies, documentation in EPIC, and collaboration with partner programs.
