Case Manager (Brooklyn

The Case Manager  provides direct case management services while engaging homelessindividualswith appropriate concern for their social, mental health, substance abuse and medical problems in order to promote client stability and empower them to reach their fullest potential.

• Completes intakes for new clients and providesanorientation to the program and shelter rules and regulations
• Reviews logbooks atthestart of each shift toensure that all clients follow-upsareconducted as required
• Participates as part of a team and regularlycollaborateswith multidisciplinary staff to monitor clients’progress
• Developsa comprehensive psychosocial assessment to fully identify and address theclient's needs
• Collaborates with internal staff to monitor clients’ medication compliance and address episodes of non-adherence
• Establishes an individualized intervention/treatment plan or strategy to promote identified goals
• Provides advocacy for individual clients around entitlements and services
• Provides case management, including individual counseling, crisis intervention, conflict resolution, advocacy,psychoeducation, coordination of services and appropriate follow-up on all referrals
• Uses motivational interviewing and seeks safety modalities to counsel clientsto improve overalleffectiveness
• Encourages and provides ongoing support so that each client might assume an active role in his or her own treatment plan tothe maximum extent possible
• Facilitates clinical, educational, supportive and/or recreation groups to address problem areas that clients experience in working towards independent living
• Maintains required documentation in case records,in written and computer databases(SCIMS, AWARDS) andupdatesclientrecords in accordance with governmental and agency policy
• Completes Housing applications (both written and computer generated) for clients based on their individual needsandfunctioning
• Escort clients as needed.
• Facilitates and assists with clients' transfers into independent living
• Participates in weekly case conferences with both internal departments and externalproviders
• Participates in regular supervision, team meetings,case conferences and staff trainings
• Maintainsa flexible scheduleincludingevening and weekend shifts
• Performs other duties asit relates to case management

• Bachelor degree in a Social Work, Psychology,Counselingoranotherrelated field preferred

• Three (3) to five (5) yearsofcase management experience with the homeless, mentally ill, substance abuse and/orlow-income populations preferred

• Strong relationship-building skills required
• Ability to work independently but know when to raise issues tothesupervisor
• Excellent organizational and time management skills necessary
• Ability to effectively multi-taskandfollow upon a timely basis
• Strong PC skills required including Word and Excel

• Bilingual in English and Spanish strongly preferred
