Job SummaryAs a professional member of an interdisciplinary team, the Social Worker provides a range of direct and indirect services for patients and families, including the identification of psychosocial problems and their resolutions. The Social Worker collaborates with the patient, family, and team to develop safe and timely discharge and treatment plans and adheres to the Mission Statements of Montefiore and Social Services, as well as the Code of Ethics of the National Association of Social Work.
EssentialFunctions
- Screen patients whose diagnosis and social data indicate need for social service intervention, according to Social Work practice standards.
- Make psychosocial assessments using all available data, including interviews with patient and family and consultation with interdisciplinary associates.
- Ensure continuity of care through development, implementation, and validation of discharge/treatment plans in a timely manner and in collaboration with patient, family, and interdisciplinary team.
- Function independently while utilizing supervision to identify problems in expediting discharge/treatment plans.
- Maintain professional conduct and practices consistent with Social Work standards.
- Ensure confidentiality in accordance with hospital and Social Services policies.
- Develop ongoing linkages and networking relationships.
- Participate in patient‑focused conferences.
- Participate in internal and external conferences.
- Demonstrate leadership by using psychosocial expertise to assist with team building, discharge/treatment planning, conflict resolution, and psychosocial education for interdisciplinary staff when appropriate.
- Participate in departmental programs such as quality improvement, statistical reporting, continuing education, staff meetings, and other related duties.
- Demonstrate knowledge of legal and ethical issues related to end‑of‑life care, including informed consent, right to refuse treatment, withholding or withdrawing life‑sustaining treatment, nutrition and hydration, DNR order, advance directives, decisional capacity, surrogate decision making, and doctrine of futility.
- Demonstrate skill and knowledge related to the identification, reporting, and appropriate intervention for victims of individual and family violence, rape, mugging, elder abuse, and child abuse/neglect.
- Master’s degree in Social Work from a program registered by the NYS Dept. of Education or deemed substantively equivalent.
- 1‑3 years of relevant work experience.
- Licensed Master Social Worker (LCSW). If holding a limited NY State permit, an executed Memorandum of Understanding with a licensed Social Worker and LMSW status within 6 months of employment is required.
- Ability to operate a personal computer and phone.
