As a Residential Plan Developer, you’ll help empower people we support with intellectual and developmental disabilities to live meaningful, self-directed lives. In this rewarding role, you’ll work closely with individuals, families, care managers, and support teams to develop person-centered plans that reflect each person’s goals, strengths, and choices. If you’re passionate about advocacy, collaboration, and helping others succeed, we’d love to have you on our team.
Location: Hudson, NY
Pay Rate: $20.50 per hour

Key Responsibilities- Develop meaningful, person-centered Staff Action Plans that support each individual’s Life Plan, goals, and personal choices.
- Partner with people we support to encourage self-advocacy, independence, personal growth, and community inclusion.
- Participate in Life Plan meetings and collaborate with care managers, families, residential staff, and other members of each person’s Circle of Support.
- Review Life Plans to ensure accuracy and communicate support needs with program staff and management.
- Maintain accurate documentation, records, and correspondence in accordance with agency policies and regulatory requirements.
- Build positive, trusting relationships with people we support by providing encouragement, advocacy, and respectful guidance.
- Serve as a liaison between individuals, families, care managers, residential providers, day services, and other support professionals.
- Provide hands-on support as needed to ensure each person’s goals and support plans are effectively implemented.
- Uphold the NADSP Code of Ethics while promoting dignity, choice, inclusion, and respect for every person we support.
