The Community Care Specialist (CCS) partners with school districts to help students and families access essential services that promote academic success, personal well-being, and stability. Acting as a primary liaison between the student, family, school personnel, and community partners, the CCS develops individualized support plans and coordinates multidisciplinary care to help students achieve measurable goals.
Key ResponsibilitiesDirect Student & Family Support
- Serve as Central Liaison:
Act as the primary point of contact for students and families, building strong, trusting relationships and facilitating clear communication across school and community networks. - Provide Family Support & Crisis Intervention:
Conduct regular home visits, offer ongoing family guidance, and provide immediate crisis intervention when urgent needs arise. - Guide Transitions:
Provide dedicated support and guidance to students and families navigating educational or life transitions.
Care Coordination & Service Delivery
- Develop Support Plans & Coordinate Care:
Create tailored, individualized support plans for students facing academic, behavioral, social, or emotional challenges, bringing together multidisciplinary teams to manage care effectively. - Facilitate Community Connections:
Partner with local agencies, service providers, and specialized student support networks to link families directly with essential resources.
Administration & Compliance
- Manage Documentation & Confidentiality:
Maintain meticulous, up-to-date student records and documentation in strict compliance with state regulations, district policies, and privacy standards.
