TAG Med Staffing is hiring a Social Worker (MSW) in New York, NY for our client, supporting case management, behavioral health, housing stability, and community-based services for adults in supportive housing. This temp-to-permanent opportunity is ideal for new graduates or early-career social workers seeking hands-on clinical experience, structured mentorship, and a clear pathway toward professional licensure. The role offers a manageable caseload while providing opportunities to develop clinical skills and advance within the organization.
Responsibilities as the Social Worker (MSW):- Case Management: Conduct resident assessments, develop individualized service plans, and provide ongoing support to promote independent living.
- Care Coordination: Connect residents with medical, behavioral health, housing, and community resources while monitoring progress and coordinating follow-up care.
- Crisis Intervention: Respond to behavioral health and medical crises, assist with hospital discharge planning, and provide appropriate interventions during emergency situations.
- Resident Advocacy: Support residents in accessing public benefits, healthcare services, transportation, and other essential community resources.
- Clinical Documentation: Maintain accurate case notes, service plans, incident reports, and other required documentation in accordance with regulatory standards.
- Resident Support: Facilitate life skills coaching, clinical groups, and relationship-building activities that promote wellness, housing stability, and overall quality of life.
- Education: Master's degree in Social Work (MSW) from an accredited institution is required.
- Experience: Previous experience working with older adults, individuals experiencing homelessness, behavioral health populations, or other vulnerable communities is preferred. New graduates are encouraged to apply.
- Industry Knowledge: Knowledge of case management, public benefits, housing resources, behavioral health services, and community support programs is preferred.
- Technical
Skills:
Proficiency with Microsoft Office and electronic documentation systems is preferred. - Skills & Attributes: Strong communication, organizational, interpersonal, and advocacy skills with a genuine interest in supporting older adults and helping individuals achieve long-term stability and independence.
