Care Manager – Flexible Caseload & Competitive Pay:
We are seeking a dedicated HH Care managers to provide individualized support and coordination for clients. This role offers a meaningful opportunity to make a difference while maintaining a flexible caseload.
Responsibilities:- Develop and implement care plans tailored to client needs
- Coordinate services and resources to ensure optimal support
- Maintain accurate and timely documentation
- Communicate effectively with clients, families, and providers
- Experience in care management, social work, or a related field preferred
- Strong organizational and communication skills
- Ability to manage multiple cases efficiently
Must have a BA
Job Types: Full-time, Part-time
Pay: $100.00 - $4,000.00 per month
Schedule:
- Monday to Friday
- Weekends as needed
Locations:
Middletown, Bronx, Newburg, Monroe, Woodbury, Goshen, Beacon, Warwick, Queens, Rockland and Westchester and Surrounding areas.
