Position SummaryWe have an exciting opportunity to join our team as a Manager-Family Support

In this role, the successful candidate Responsible for the development and implementation of a comprehensive and coordinated system of Family Support Services within the context of a health care delivery system and in collaboration with appropriate internal and external stakeholders. Family Support Services is inclusive of case management,short term supportive counseling, advocacy, coaching, benefit application assistance, and parenting support. This position is responsible for the integration of these services within clinical and community settings and developing intervention strategies that are sustainable across multiple funding and financing strategies.
Overall responsibilities include planning and executing new initiatives, supervision of FSS supervisors, reviewing documentation and data to ensure continuous quality improvement, completion of contractual reports, developing resources and fostering community partnerships.
- Ensure that supervisors have the tools needed to assess and support their supervises
- WorkwithMCIT,thedatadepartment,andotherFHCstakeholderstoevaluatetheeffectivenessofSDOH screening projects and identify continuous improvement plans.
- Ensure that referred clients receive needed services
- Facilitate weekly staff meetings
- Assist in the preparation of required reports for FSS grant funded initiatives.
- Monitor progress towards meeting grant deliverables and make adjustments in programming to ensure that outcomes are met.
- AssistinthecoordinationofandimplementationofstaffdevelopmentforFSSteammembersandother Community Based Programs staff.
- Ensure that staff are up to date on benefits and resources needed to support clients.
- Use data as a tool for program development evaluation and continuous improvement
- Identify best practices and work toward the development and implementation of evidence-based models of care/programmingthatmeetstudent/familypsychosocialneedsinordertoachievebettermeasurablehealthand educational outcomes.
- Review documentation to ensure that it meets department standards.
- Assure that programs are addressing necessary objectives to fulfill contract requirements.
- Collaboratewithothercommunitybasedorganizationstoprovidecomprehensiveservicestoindividualsand families.
- Provide ongoing evaluation and assessment and coordinate staff development and training.
- Assessnewprogramopportunitiesandidentifyanddevelopnewinitiativestobettermeetthechangingneedsof the community
- Work with social care networks to ensure that staff are effectively using the 1115 Medicaid initiative to help clients access needed services.
- CollaboratewithITAnd Quality Assurancetousetechnologytoeffectivelyscreen ,referandaddresspatient needs.
- Work to ensure sustainability of FSS department through billing opportunities and exploration of funding opportunities.
- To qualify you must have a B.A.
- 3 years of managerial or project coordination experience within complex and integrated systems of care
- Excellent organizational and communication skills
- Competency in partnering with cultural diverse communities
- Bilingual in Spanish, Mandarin or other language spoken in community.
- Master's in Social Work
