TAG Med Staffing is hiring a Care Coordinator (Social Worker) for our client in Brooklyn, NY to support a Medicaid-funded care coordination program. This temporary position will focus on conducting client screenings, coordinating referrals, and connecting individuals with health-related social services while collaborating with providers and community organizations. The ideal candidate has case management experience, strong knowledge of New York City social service resources, and is comfortable working independently in a technology-driven environment.
Responsibilities as the Care Coordinator (Social Worker):
- Care Coordination: Conduct client screenings and assessments, identify health-related social needs, and coordinate referrals to appropriate community-based services and support programs.
- Case Management: Maintain ongoing communication with clients, providers, and community organizations to monitor service delivery, track outcomes, and ensure timely follow-up.
- Resource Navigation: Connect individuals with Medicaid-funded programs, social services, housing, food assistance, transportation, and other community resources based on identified needs.
- Documentation: Maintain accurate client records, document case activity, update referral information, and ensure all services are tracked within the care management platform.
- Collaboration: Partner with internal teams, healthcare providers, and community agencies to support coordinated care and positive client outcomes.
- Compliance & Operations: Adhere to program requirements, confidentiality standards, and organizational policies while supporting quality improvement initiatives and completing required training.
Qualifications for the Care Coordinator (Social Worker):
- Education: Bachelor's degree in Social Work, Human Services, Psychology, Public Health, or a related field is required.
- Experience: Minimum of 2 years of experience in case management, care coordination, care navigation, social services, or a related field is required.
- Industry Knowledge: Experience working with healthcare, Medicaid, managed care, community-based organizations, or social service programs is required. Experience supporting value-based care or Medicaid waiver programs is preferred.
- Technical
Skills:
Proficiency with Microsoft Office and electronic care management or case management systems is required. Experience with care coordination platforms is preferred. - Skills & Attributes: Strong organizational, communication, and problem-solving skills with the ability to manage multiple priorities, build relationships with clients and community partners, and work effectively in a fast-paced environment.
