The West Side Federation for Senior and Supportive Housing, Inc. (WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops, manages, and provides housing for low‐ income older persons, many of whom live with mental illness and/or have experienced homelessness.

PositionDescription
The Case Workeris expected to work under the guidance ofthe Clinical Coordinator. They will develop 1:1relationshipswith the residents which will enable them to provide arangeservices to the residents which enable the residents toestablishand maintain safe and independent lives in the community.
Please Note:
This is a full‑time position (5) days a week on‑site.
- Maintainaccurateand organized case files, records, and reports.
- Write progress notes after each resident encounter.
- Develop individualized service plans with residents.
- Draft correspondence, notices, and reminders for residents as needed.
- Completedataentryof resident progress in internal databases.
- Assist residents with general inquiries or referrals to services.
- Accompany residentstoappointments or community activities as needed.
- Helpfacilitatesocial and recreational activities for residents.
- Assist in preparing materials for group sessions or workshops.
- Carrya caseload of residents whopresentless challenging.
- Participate in team meetings andtrainings.
- Assistsocial workers in emergency situations as directed.
- Report urgent resident concerns promptlytosupervisingsocial workers.
- Good written and oral communication skills.
- Solid organizational ability, time management, and interpersonal skills.
- Basic computer skills and familiarity with data entry.
- Desire or interest in working and supporting an aging population.
- High school diploma or equivalent;associate or bachelor'sdegree in social work, Human Services, or related field preferred.
