Location: New York

  • Provides trauma-informed therapeutic case management and support to children, youth, and families involved in foster care.
  • Partners with families to develop individualized service plans, promote safety and permanency, and provide crisis intervention.
  • Coordinates care with internal and community providers, implements evidence-based practices, and advocates for youth through court appearances and ongoing case management.
  • Monitors safety, well-being, and permanency progress by conducting regular casework contacts and providing necessary services.
  • Completes timely documentation of all reports and casework activities including FASPS, Psychosocial, Adoption Home studies, and Kinship Home studies.
  • Actively participates in Family Court Hearings and Family Team Conferences to advance permanency for children and families.
  • Provides family planning counseling to youth and documents discussions with foster parents.
Requirements
  • Bachelor's Degree required; BSW preferred.
  • Experience supporting families and adolescents with social services.
  • Must have a commitment to work from a strength-based, youth development and trauma informed perspective.
  • Ability to work with diverse population of clients and team members.
  • Must be highly organized with ability to meet program deliverables and deadlines.
  • Must have excellent verbal and written communication skills.
  • Ability to work flexible hours - some evenings or early mornings as needed to meet client needs.
  • Ability to use public transportation to travel throughout New York City or Agency vehicle when available.
