Bronx Works is seeking a dedicated Case Manager to join Park Haven, a supportive housing program in the South Bronx. You will manage a caseload, conduct home visits, and document progress toward treatment goals, working within a Housing First model to connect clients with essential services.
The role requires strong writing and communication skills, a Bachelor's in social work, and knowledge of city resources. Schedule is Mon-Fri 9am-5pm with on-site responsibilities.
