Overview

Reporting to the Director of Violence Prevention, the Hospital based Intervention Program (HVIP) Case Manager plays a critical role in helping patients affected by violence move from crisis toward safety, stability, and long-term recovery. Working as part of One Brooklyn Health’s Hospital-Based Violence Intervention Program at the Brookdale campus, the Case Manager provides intensive, trauma-informed case management to patients and families during hospitalization and throughout their transition back into the community.

The position builds trusted relationships with individuals at a vulnerable and often life-changing moment, conducting comprehensive assessments, developing individualized service plans, coordinating hospital and community-based resources, and helping clients address barriers involving personal safety, housing, employment, education, behavioral health, healthcare access, and other social determinants of health.

As part of New York City’s Crisis Management System, the program uses a public-health approach to reduce reinjury, prevent retaliation, and address the underlying conditions that contribute to community violence. The Case Manager works closely with hospital staff, violence-prevention professionals, community partners, and service providers to support each client’s recovery and promote safer, healthier communities.

Responsibilities
  • Provide ongoing case management, including follow-ups, advocacy, and referrals to community-based resources.
  • Conduct comprehensive assessments to identify each client’s immediate and long-term needs, including personal safety, housing, employment, education, behavioral health, healthcare access, legal-service referrals, and family or social-support needs. Develop individualized, client-centered service plans in collaboration with clients, establishing appropriate goals, interventions, referrals, and follow-up activities. Coordinate services with physicians, nurses, social workers, care managers, behavioral health professionals, and other hospital personnel to support discharge planning and continuity of care.
  • Advocate for clients when navigating healthcare, social-service, educational, employment, and community-service systems, within the scope of the position. Connect clients with appropriate hospital, governmental, and community-based resources, including counseling, education, employment readiness, housing assistance, benefits, legal-support services, and other stabilization programs. Maintain complete, accurate, timely, and confidential case records in accordance with program and hospital requirements.
  • Track client progress, outcomes, and service utilization.
  • Participate in data collection needed for grant reporting, and program evaluation.
  • Provide crisis intervention, conflict de-escalation, and immediate supportive assistance in accordance with program protocols and professional boundaries.
  • Assist clients in developing individualized safety plans intended to reduce any risk of retaliation, possible reinjury, or further harm.
  • Develop and maintain effective working relationships with community-based organizations, violence-prevention partners, government agencies and other service providers.
  • Participate in community outreach activities, violence prevention initiatives, educational events, and other program-related activities as assigned.
  • Participate in interdisciplinary meetings, case conferences, supervision sessions, staff development activities, and program planning meetings.
  • Contribute to program development, workflow improvement, service evaluation, and quality improvement initiatives.
  • Perform other related duties consistent with the responsibilities and qualifications of the position.
Qualifications Education
  • Bachelor’s degree from an accredited college or university in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Public Health, Sociology, or a closely related field.
  • Associate degree from an accredited college or university in Human Services or a closely related field, combined with additional directly related professional experience.
  • Master of Social Work or another relevant graduate degree is preferred
