Case Manager – Immediate Full Time Opportunity Bronx, New York
River Spring Living is seeking a Case Manager to join our dynamic team for our Supportive Housing Community!
The Case Manager role for our Arthur Avenue Apartments located in the East Tremont/Arthur Avenue section of the Bronx.
Position OverviewWith the mission to promote “aging in place,” the Case Manager position will holistically address and support tenant’s medical, social and community needs in an effort to reduce the cycle of avoidable hospitalizations and over utilization of emergency departments as outlined in our ESSHI work plan. Under the supervision/guidance of the Group Services Supervisor, this role will provide supportive services to all seniors within the River Spring ESSHI program.
Key ResponsibilitiesUpholding the program’s policies and procedures which create a case management system.
Will assist in the ongoing assessment of a tenant’s strengths and needs, crisis management/intervention, provides or arranges for the provision of identified services/resources, monitors the receipts of such services, evaluates and reports their effectiveness in improving tenant functioning and quality of life, as well as documents all activities and outcomes.
Responsibilities also include providing tenants with as needed emotional and social support, education on entitlements, benefits and supports tenant in maintaining such benefits.
Will work to build relationships with appropriate outside agencies as needed to support tenant and program’s needs.
QualificationsBachelor’s Degree required, preferrable in Social Work or in a healthcare related field, plus one year of related experience.
Formal education must have included completion of a course of study in one of the following areas but not limited to: social work programs, gerontology studies, human services for dependent adults, human resources or home economics.
Related work experience may include but need not be limited to residential care, rehabilitation, health care management and social work.
Compensation$30.2197 min - $35.7142 max. Hourly
