Job Overview – Geriatric Care Manager (Social Worker)
Schedule:
Monday to Friday (Hybrid)
TAG Med Staffing is hiring a Geriatric Care Manager (Social Worker) in Queens, NY for our client, supporting care management, case coordination, and advocacy services for older adults living in the community. This hybrid role combines field-based home visits with remote administrative work and weekly in-office collaboration to help seniors maintain independence, access essential resources, and improve their overall quality of life.
The ideal candidate will have experience working with older adults, strong case management skills, and a compassionate, client-centered approach.
- Care Management: Manage a caseload of older adult clients by conducting assessments, developing individualized care plans, and monitoring ongoing needs.
- Client Advocacy: Assist clients in navigating healthcare, housing, financial, and community resources while advocating for appropriate services and support.
- Home Visits: Conduct regular in-home visits and maintain ongoing communication with clients, families, and care providers to ensure continuity of care.
- Care Coordination: Collaborate with healthcare professionals, family members, and community organizations to coordinate services and support positive client outcomes.
- Documentation & Compliance: Maintain accurate case documentation, service plans, assessments, and progress notes in accordance with program requirements.
- Team
Collaboration:
Participate in case conferences, team meetings, and quality improvement initiatives while contributing to a collaborative care management environment.
- Education: Master's degree in Social Work or a related human services field is required.
- Licensure: LMSW or LCSW is preferred.
- Experience: Experience in care management, case management, social work, or supporting older adults or other vulnerable populations is required.
- Industry Knowledge: Knowledge of community-based services, aging services, healthcare systems, benefits programs, and care coordination is preferred.
- Technical
Skills:
Proficiency with Microsoft Office Suite and electronic documentation or case management systems is preferred. - Skills & Attributes: Strong communication, organizational, advocacy, and interpersonal skills with the ability to work independently, manage a caseload, and build meaningful relationships with clients and community partners are required.
