Location: New York
Catholic Charities Brooklyn and Queens, Inc. is seeking a Case Manager II to provide assessment, intake, crisis intervention, information and referral, and comprehensive case management for clients at risk of homelessness.
Under the supervision of the Supervisor, you will maintain case records, arrange and follow up on referrals, assess needs, and conduct home/field visits across the five boroughs as needed.
