Location: New York

The Medicaid Specialist is responsible for guiding caregivers through the Medicaid eligibility and enrollment process, ensuring they receive the benefits and long-term care services for which they qualify.

As a key member of the Caregiver Support team, the Medicaid Specialist prepares and submits Medicaid applications and provides ongoing support throughout the application process.

Assessment may be conducted in the SCS office, in the caregiver/care receiver’s home, and any other designated location convenient for the caregiver and care receiver.

In addition to Medicaid responsibilities, the Specialist will collaborate with program staff on outreach initiatives to increase awareness of the program, strengthen community partnerships and connect caregivers with available resources.

The Caregiver Support, Alzheimer’s Services Program provides personalized support, education, care planning and resources to individuals and families caring for someone living with Alzheimer’s disease or some other form of dementia.

Responsibilities
  • Assess clients’ financial status and eligibility for Medicaid and other public benefits and entitlements.
  • Educate caregivers on Medicaid eligibility requirements, enrollment procedures, and available long-term care options.
  • Complete, submit and monitor Medicaid and entitlement applications, ensuring all required documentation is accurate and submitted in a timely manner.
  • Maintain regular follow-up with caregivers to monitor application status and obtain necessary documentation.
  • Maintain accurate client records, progress notes, and required documentation in accordance with program and regulatory requirements.
  • Build and maintain relationships with community organizations, healthcare providers, and referral sources to support program visibility.
  • Participate in community events, educational programs and conduct presentations to promote the Caregiver Support Program.
  • Participate in interdisciplinary team meetings and case conferences, as appropriate.
Qualifications
  • AA/AS degree and a minimum of 2 years of experience.
  • Knowledge of federal, state, and city regulations related to Medicaid eligibility and entitlements.
  • Bilingual - Spanish, Mandarin or Cantonese required.
  • Effective written and oral communication skills.
  • Experience in a nonprofit organization is preferred.

Salary: $46,000- $48,000 annually (non-exempt)
