Urban Health Plan seeks a dedicated Case Manager for Adult Medicine Social Services in the Bronx. The role supports patients with social services, referrals, and advocacy, focusing on adult medicine at our Bronx-based health centers. A Bachelor’s in a related field and bilingual English/Spanish are required.
Work is onsite, Monday–Friday, 8:00 am–5:00 pm. Salary is $45,560 with comprehensive benefits including health insurance, retirement plan, paid time off, and PSLF eligibility.
