Location: New York
Jericho Project in New York City seeks a Life Coach to provide case management and service coordination at the Melrose 78-unit Permanent Supportive Housing program. The role requires experience with adults facing mental illness and substance use histories, with weekly to monthly contact with colleagues.
The Life Coach will conduct assessments, develop care plans every six months, and coordinate with psychiatrists and other providers.
