Position Summary The HOPE Program is seeking a Case Manager who will engage with training participants and alumni in both one-on-one as well as groups setting to provide counseling, guidance, and referral, and case management services to a case load of 120 to 150 individuals, including both low and high touch cases. The position receives daily supervision from the Senior Case Manager and reports to the Director of Client Engagement.
The Client Engagement team as a whole is responsible for meeting the overall recruitment, admissions, and case management goals resulting in enrollment, completion, placement, and retention outcomes in line with HOPE’s strategic plan. In doing so, the team will assess training candidates for proper fit in order to commit to HOPE’s full-time training program as well as provide wrap-around services, wellness support and case management to address barriers that may impede their ability to successfully complete training and secure employment.
The Case Manager is expected to work a minimum of four days from the office across all HOPE and partner locations and work schedule will include evening and weekend hours.
Responsibilities Provide comprehensive, participant-centered case management services to individuals experiencing barriers to employment and self-sufficiency, including participants in training, participants in job search phase, and alumni.
Maintain regular contact with participants through individual meetings, phone calls, virtual appointments, and classroom visits to monitor progress and provide ongoing support.
Utilize trauma-informed, strength-based, and motivational interviewing techniques to promote participant engagement, empowerment, and goal attainment.
Provide crisis intervention, conflict resolution, and supportive counseling while responding appropriately to participant needs.
Connect participants to community resources and supportive services, including housing, public benefits, healthcare, mental health services, childcare, legal assistance, food resources, and workforce development programs.
Maintain current knowledge of community resources, New York City workforce systems, HRA, and community-based organizations.
Contribute to HOPE’s working database of social service partners and resources in areas, such as public benefits, affordable housing, childcare, pro bono legal services, basic needs access, etc.
Maintain and cultivate partnerships for social service referrals for students that are warm and human-centered, recognize the needs of participants and capacity of the provider, and offer a feedback loop to HOPE.Collaborate with instructors, employment specialists, recruitment staff, and other program personnel to support participant retention and successful employment outcomes.
Facilitate workshops, orientations, and group discussions related to wellness, life skills, employment readiness, community resources, and other participant development topics.
