Location: New York
Breaking Ground seeks a Case Manager, Connect to Care PFNYC to provide outreach and case management for people experiencing homelessness. You will assist clients in transitioning to permanent housing, conduct psychosocial evaluations, and connect them with psychiatric and medical services.
Reporting to the Social Services Supervisor, the role includes advocacy, record-keeping, and coordination with service providers. This is a 35-hour per week, on-site position in New York.
