Established older adults program provides comprehensive services to the diverse population of the Lower East Side. These services incorporate the Department for the Aging Health and Wellness model into the daily activities. Services include congregate and homebound meals, case assistance, social and recreational activities. Ongoing training and professional supervision provided.

• Perform intake on-going assessment,provide services they need to ensure good health and wellness
• Provide outreach, information, case assistance, entitlement evaluation,referrals and advocacy
• Document services provided to clients in writingandkeep client files up to date; provide regular statistical reporting on outcomes.
• Coordinate with Longevity through Education Art Recreation and Nutrition (LEARN) staff for quality control and integration of services
• Assistin designing documents or forms for record-keeping and outreach purposes;assistin program planning and evaluation
• Assist with program activities
• Complete data entry intoappropriatedatabasesystemsand CRMplatformstimely
• Attendmeetingsandprofessional development trainings
• Keepup to dateon all information and resources pertaining toolder adults
• Collaborate with program staff and supervisors
• Other duties as assigned byProgram Director

• Bachelor's degree in social work, Human Services, or related field required
• At leastoneyearsofexperienceworking with adult population
• Strong interpersonal skills including tact, diplomacy, patience, and professionalism
• Working knowledge of social theories and practices
• Social perceptiveness and empathy
• Ability to build andmaintainprofessional helping relationships
• Ability to relate and communicate with diverse population and groups
• Resilience along with ability to assess situations
• Excellent skills in Microsoft Office
• Must have strong attention to detailand able to work independently
• Mustdemonstratestrong commitment to working in a multi-cultural environment
• Demonstrated ability to engage hard to reach populations
• Excellentverbalandwritten skills required
• Extensive knowledge of NYC benefits programs and advocacy skills required
• Bilingual language skills required,must be fluent in written and spoken language requirements will change based on needs.
• Will berequiredto obtain CPR/AED certification upon hire

Work Schedule:

Full time, Monday, Wednesday, and Friday, 11:30am - 7:30pm, Tuesday and Thursday, 9:00am - 5:00pm, 35 hours per week
