Location: New York
POD Health seeks a Nurse Practitioner to deliver comprehensive care via telehealth and in-person visits. You will manage chronic and acute conditions, oversee RPM monitoring, and contribute to CCM and CHI initiatives from our Brooklyn base.
Responsibilities include patient assessments, treatment planning, and collaboration with interdisciplinary teams to provide patient-centered care, with a Monday–Friday schedule and home base in Brooklyn, NY 11218.
