Services for the Under Served is seeking a Wellness Coach to assist agency consumers in accessing needed community services, developing referrals, and following up to ensure access. You will support case management, service planning, crisis intervention, and advocacy as part of a collaborative team.
The role requires a Bachelor's degree and proficiency with MS Office; bilingual English/Spanish is preferred. Work is in-person at our New York service sites with opportunities to grow professionally.
