Across our 60+ locations, we provide programs in a variety of service areas, including workforce development, children, youth, families, seniors and homeless prevention.

The Pyramid Safe Haven is a 125-bed, low-barrier shelter operated by Bronx Works for chronically street-homeless single adult men. Located in the Melrose section of the Bronx, Pyramid follows a Housing First model and provides on-site services including case management, psychiatric and medical care, substance use counseling, meals, showers, and housing placement support. Referrals are made exclusively through NYC’s Street Homeless Outreach Teams.

The program focuses on building trust and engagement while supporting clients in transitioning into stable, long-term housing.

RESPONSIBILITIES
  • Independently manage a caseload
  • Conduct regular field visits to engage and build rapport with chronically homeless individuals.
  • Work with clients to develop goals and service plans.
  • Provide referrals and direct assistance to support clients applying and/or attending appointments
  • for mental health services, medical care, substance treatment, vital documents, benefits,
  • legal/immigration services, and permanent housing.
  • Document all interactions with or on behalf of clients, monitor and document client progress
  • Maintain familiarity with social services and resources available both on-site and in the
  • community.
  • Coordinate client care with other service providers; attend regular case conferences with
  • clients and other service providers, as required.
  • Attend regular supervision sessions with direct supervisor.
  • Drive agency vehicle to and from client locations and appointments
  • Perform additional duties as assigned.
QUALIFICATIONS:
  • Bachelor’s degree in social work or a related field of study
  • Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3
  • years, candidate must have obtained no more than 3 license violation points
  • One-year of case management or related work experience preferred.
  • Ability to engage and build rapport with populations with high support needs.
  • Strong oral and written communication skills
  • Detail-oriented and resourceful with strong time management and organizational skills
  • Proficiency in Microsoft Office suite and other standard business technology
  • Bilingual/multilingual preferred, but not required.
  • Valid driver’s license is required for this position.
PHYSICAL REQUIREMENTS
  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public
  • transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of
  • weather, including inclement weather, if required to conduct home visits and/or field work
  • TB Test required within the first 120 days of employment.
