Location: New York
Bronx Works is seeking a full-time Case Manager in Bronx, NY to independently handle a caseload in a shelter-focused program. You will engage chronically homeless individuals through regular field visits, help develop goals and service plans, and connect clients with housing, health, and benefits resources.
The role requires a Bachelor’s degree in social work or related field, a valid driver’s license, and strong communication skills. Prior case management experience is preferred.
