Community Housing Innovations is seeking a Case Management Supervisor in Brooklyn, NY to lead a team of Case Managers and coordinate service and care plans for residents. You will ensure plans are up to date, customize to needs, and build partnerships with community providers to support stabilization and independence.
Pay: $66,300 annual.
Schedule:
Tuesday, Thursday – Saturday 8am–4pm;
Wednesday 11am–7pm.
Location:
Sheepshead Bay, Brooklyn, NY.
