This role reports directly to our Senior Vice President of Customer Experience and is a critical part of Epicured’s New York Medicaid Section 1115 Waiver program, supporting Medicaid members through screening, navigation, and connection to eligible services.

Role Overview

Epicured is seeking Social Care Navigators
(Hybrid) to support our expanding New York Medicaid Section 1115 Waiver initiative. This program is designed to improve health outcomes, expand equitable access to care, and reduce health disparities by helping Medicaid members receive support for health-related social needs (HRSN) and qualifying nutrition-related services.

In this role, you will conduct outreach and screening with Medicaid members telephonically to assess eligibility for Epicured services and help navigate members to internal and external resources they qualify for. You will use designated contact center and program platforms to accurately document screening outcomes, referrals, and next steps.

This position is ideal for someone who is highly organized, compassionate, tech-savvy, comfortable in a high-volume call environment, and motivated by supporting members with complex needs.

Key Responsibilities

  • Manage inbound and outbound member communications within a virtual Five9 contact center environment (phone and email) in accordance with service level and quality standards
  • Conduct member outreach and engagement following referral intake to support connection to services
  • Assist with scheduling and coordination of non-clinical assessments and follow-ups
  • Navigate members to medically tailored nutrition services and other HRSN-related supports
  • Facilitate referrals to internal programs and external community-based organizations (CBOs) and SCN partners
  • Support closed-loop referral processes, including tracking status, follow-up, and confirming service linkage
  • Maintain accurate, timely, and compliant documentation within the SCN platform and internal systems
  • Provide member support for general inquiries, program participation, and service navigation
  • Adhere to all applicable Medicaid, HIPAA, and NYS DOH privacy and documentation requirements

Qualifications

  • High school diploma required;
    Associate’s preferred
  • 3-5 years of experience in customer service, call center operations, social services, or care navigation
  • Experience working in a virtual contact center environment (Five9 preferred)
  • Familiarity with NYS Medicaid programs, Social Care Networks (SCNs), and HRSN concepts preferred
  • Strong communication and engagement skills with a member-focused approach
  • Ability to manage high-volume interactions in a structured, compliance-driven environment
  • Bilingual (English/Spanish) preferred

Schedule Requirement:

  • Ability to work 9:00 AM – 6:00 PM EST, Monday – Saturday (specific schedule assigned based on coverage needs)

Preferred Qualifications

  • Healthcare, patient screening, member services, care coordination, navigation, or community-based program experience
  • Experience supporting members with complex needs through structured screening and referral processes
  • Multilingual candidates strongly encouraged to apply, including:
    Spanish, French, Hindi, Georgian, Mandarin Chinese, Korean
  • Familiarity with Social Care Network (SCN) platforms or workflows (highly preferred)
