- Coordinate Personalized Plans:
Develop and implement individualized plans for each client, including Staff Action Plans, Individual Plans of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, and Levels of Supervision. - Lead Interdisciplinary Teams (IDT):
Facilitate team meetings to ensure that clients receive quality active treatment and that their needs are met holistically. - Monitor Progress:
Track and evaluate the progress of clients’ goals and achievements, adjusting plans as necessary. - Ensure Compliance:
Maintain accurate, up-to-date electronic records and ensure all documentation is in compliance with OPWDD (Office for People With Developmental Disabilities) regulations. - Support Independence:
Assist clients in gaining independence with life skills, social interactions, and other personal goals. - Provide Input for Training Programs:
Help develop and implement training programs for staff and clients, monitoring progress and outcomes.
Educational Requirements
Bachelor’s degree in a Human Services field (or related field) required.
Experience & Skills- 1+ years of relevant experience in a similar role preferred or 1 year of DSP experience
- Proficiency in person-centered planning, instructional methods, and progress tracking
- Strong organizational, communication, and interpersonal skills
- Valid New York State driver’s license is required
- Understanding of OPWDD regulations and the ability to maintain accurate documentation
