Program Background

Family Support Programs provide family‑focused assistance utilizing a Solution‑Based Casework model that emphasizes goals to stabilize and support families. The approach promotes skill acquisition and behavior change, aligned with the promotion of child well‑being outcomes.

Position Summary

The Family Support Case Planner supports and stabilizes families involved with child welfare or in crisis, utilizing the Solution‑Based Casework model to prevent out‑of‑home placement. Case‑work counseling is provided and/or referred to address issues concerning intimate partner violence, mental health concerns, substance use, and school difficulties. Home visits are required to assess child safety.

Required Qualifications
  • Bachelor’s degree in a related field (Social Work, Psychology, Mental Health)
  • 1+ years of related work experience with children and/or families preferred but not required
  • Excellent communication skills (verbal and written)
  • Proficiency in Microsoft Word and Outlook
  • Ability to engage with clients
  • Knowledge and training in working with children and families, including child development stages and family dynamics
  • Willingness to make home visits and work various hours (no weekends)
  • Understanding and willingness to commit to the agency’s mission, core values, Sanctuary Commitments and Diversity, Equity, Inclusion, and Belonging (DEIB)
Responsibilities
  • Support and stabilize families involved with child welfare or in crisis
  • Utilize Solution‑Based Casework to prevent out‑of‑home placement
  • Provide case‑work counseling and refer to services for intimate partner violence, mental health, substance use, and school difficulties
  • Conduct home visits to assess child safety and family circumstances
  • Engage with clients and maintain accurate case records
  • Adhere to agency policies, core values, and DEIB commitments
  • Document goals, progress, and outcomes in agency systems
  • Collaborate with community partners to support families and children

Salary Range

$48,500 to $53,500 per year
