Case Manager

• Provide care coordination and case management for a caseload of 20–25 clients who are aging or living with a disability
• Conduct client initial intakes and monthly and annual assessments
• Monitor clients’ conditions and services through monthly in-person or virtual visits, and additional calls, emails, or texts as needed
• Coordinate and participate in medical and end-of-life decisions in consultation with legal staff and medical professionals
• Communicate and advocate for clients with agencies and institutions
• Work closely with clients’ families and loved ones
• Participate actively in team meetings and case reviews
• Initiate and coordinate pre-paid funeral arrangements
• Maintain communication with medical and home care providers
• Arrange home cleaning and extermination when needed
• Keep timely and detailed case notes and communication records
• Complete initial, annual, and final accounting reports
• Prepare administrative documentation and reports for grants/funding programs
• Organize and schedule work and assignments in Outlook calendar
• Update and maintain clients’ electronic files and records
• Screen clients for government and private benefits
• Gather information and complete and submit applications and recertifications
• Apply for and maintain clients’ medical insurance
• Travel throughout the five boroughs of NYC and potentially other regional locations to meet clients, families, and professionals

• Relevant experience and work history may be accepted in lieu of a degree

• Travel to other regional locations may be required
• Bachelor's and/or Master's Degree in Social Work or related field preferred
• Authorization to work in the United States

Proficient in organizing and scheduling tasks, managing client communications, and navigating complex benefit systems.
