Location: New York

Optimal Care Services is seeking a Service Coordinator to serve as the primary point of contact for families of infants and toddlers (birth to 3 years) enrolled in the Early Intervention Program. We believe all children are capable of learning and achieving, and as a Service Coordinator, you will guide families through the EI process, ensure timely access to services, and empower families to advocate for their child's developmental needs.

Key Responsibilities

  • Serve as the primary point of contact for assigned families throughout the EI process
  • Coordinate and facilitate IFSP meetings, ensuring family priorities are addressed
  • Monitor service delivery to ensure all authorized services begin within required timelines
  • Educate families on their rights, available services, and the EI process
  • Connect families with appropriate evaluators and service providers
  • Maintain regular contact with families to address questions and concerns
  • Collaborate with providers, EIOD, and other team members
  • Complete all required documentation in NYEIS/EI-Hub accurately and timely
  • Support families through transition planning to preschool services
  • Advocate for families and help them navigate the service system

Qualifications and Requirements

  • Bachelor's Degree in Social Work, Psychology, Education, or related Health and Human Service field required
  • Must complete Introductory Service Coordination Training to be hired
  • Strong organizational and case management skills
  • Excellent communication and interpersonal abilities
  • Ability to work with diverse families from various cultural and linguistic backgrounds
  • Proficiency with computer systems and documentation
  • Bilingual Spanish or Bilingual Bengali highly preferred
  • American Sign Language a plus
  • Recent college graduates are welcome to apply

Work Model

In-person, Monday-Friday, 8:00 AM - 4:00 PM. Eligible for one remote day per week after completing one month of employment.
