About Lehoel
Lehoel is the front door to New York's Social Care Network (SCN), funded through the state's 1115 Medicaid waiver. We screen, navigate, and connect Medicaid members to the food, housing, transportation, and care they need to be well — free, confidentially, and in their own language.
The Role
As a Social Care Navigator, you're the steady, human point of contact for the members we serve. You'll screen for needs, build personalized plans, connect members to vetted community partners, and follow up to make sure things land. Members talk to the same navigator each time — no handoffs, no scripts.
What You'll Do
- Conduct confidential screenings (15–20 minutes) on food, housing, transportation, utilities, and benefits
- Build personalized plans and make warm referrals through the Unite Us platform
- Help members access SNAP, Medicaid, WIC, HEAP, housing, and transportation support
- Follow up at 30, 60, and 90 days, adjusting plans as life changes
- Meet members by phone, video, or in person
- Maintain accurate, confidential, HIPAA-aligned records
What We're Looking For
- Strong communication, listening, and organizational skills
- Experience in case management, social services, or care coordination (preferred)
- Bilingual — English plus Spanish or Haitian Creole (strongly preferred)
- Familiarity with Rockland County community resources a plus
- A genuine commitment to health equity
