• Join our team as a Case Planner where you'll work in a collaborative environment to support children with diverse mental health needs.
• This full-time position based in the Bronx, NY, involves managing a caseload of approximately 10 children, providing intensive casework, and coordinating essential services.
• Proficiency in using generic child welfare systems is required.

• Collaborate with therapists, education specialists, and social workers.
• Manage a caseload of approximately 10 children.
• Provide intensive casework services.
• Conduct outreach to connect families with community resources.
• Coordinate Family Team Conferences.
• Maintain accurate case records.
• Participate in case consultations.

• Master's OR Bachelor's Degree in Social Work, Psychology, or related field.
• Foster Care experience preferred.
• Bilingual (Spanish/English) preferred.
• Valid Driver's License required.
• Excellent verbal, writing, and organizational skills.
