Director of Homebound Services

For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities.

Home Delivered Meals Program provides meals to the homebound elderly residing in Queens. The program provides one hot meal and one cold pack to clients to improve the quality of their lives. Meals include Kosher, non-Kosher, Halal, Caribbean, Korean, Chinese, South Asian, and Mediterranean cuisines.

The Director of Homebound Services for Older Adults will oversee all assigned case management programs and Home delivered meals programs serving homebound elderly in community. Director of Homebound Services is responsible for the day-to-day administration and operation of case management programs, oversees meals delivery operations, ensures quality services to the elderly and contractual obligations to the funding sources

• Manage the overall proper required operations of programs day by day.
• Ensure proper and timely reporting as per funding and agency requirements.
• Plan and strategize program’s core function and coordinates continuous quality improvement of services.
• Establish partnerships and linkages to agencies and resources in the community.
• Ensure client satisfaction measurements are in place.
• Ensure fiscal accountability of programs for homebound elderly.
• Provide leadership to programs’ staff to assure the timely planning and implementation of quality services to clients including client assessments, service plans and screening/referrals that meet the needs of older adults.
• Provide ongoing supervision of programs’ staff through regular individual supervisory meetings, group supervision and case reviews.
• Ensure programs and staff adherence to agency and funding source requirements.
• Provide direct services to clients in case review and crisis intervention when necessary.
• Ensure that referrals and follow ups are made to appropriate internal and external services as required to meet the consumers’/families’ needs, especially, with older adults with functional impairments helping them gain access to appropriate services to maintain their safety, health, and quality of life.
• Participate in the resolution of consumers’ complaints/ grievances.
• Initiate and implement Continuous Quality Improvement process as well as Strategic planning for case management programs.
• Maintain working relationships with other service providers in the community and represent the program and agency at meetings when requested.
• Responsible for overseeing and maintaining funding source software systems at programs according to NYC Aging
• Develop and maintain a quality assurance plan according to the policies of the funding source and agency.
• Hiring, training, and evaluating staff, in accordance with agency guidelines
• Work with and advocate to the elected officials according to funding source and agency policies for the needs of the elderly and to assist the program to meet these needs.
• Attend funding source and agency meetings, training and workshops as needed or required.

• Master’s in social work or Mental Health Counseling, Master degree in Psychology.
