Under the direct supervision of the Prevention Supervisor, the Caseworker is primarily responsible for providing case management and counseling services toidentifiedyouth and families.

Responsibilities include advocacy, referral and linkage, group, individual and familycounselingand crisis intervention.

• Utilizing the task centered case management model, implements and coordinates services to children and their family'shaving troublein the home and/or in school, aimed at preventing placement of children outside the home and/or at improving academic performance.
• Provides intervention and support to families in crisis intended to keep the family unit intact.
• Completes all required documentation, in timelinesin accordance with program standards.

• Assesses children and families for risk, develops and implements service plans to address risk factors, drafts petitions and testifies in Family Courtas necessary.
  
  Provides Foster Care Services andlocatesplacements when needed.

• Advocates on behalf of children and their families to ensure adequacy of services (i.e., educational, legal and community) by providing linkage and coordination with thecountyproviders and agencies.

• Processes and reviews intake of all referrals to program.

• Provides services in a manner which is culturally competent. Empowers clients by using clients’cultural strengths and informal support networks in service delivery.

• Maintains effective professional relationships with other service providers and community organizations in the interest ofclients’and program goals.

• Identifiesandreportson Untoward Incidents as required.

• Carries out Agency policies and procedures.

• Completes all necessary written reports, statistics, casefilesand summaries in a timely mannerandin accordance with program and agency standards.

• Attends andparticipatesin individual supervisory conferences, staff meetings, trainingsessionsand professional development opportunities as required.
  
  Maintains required program certifications.

• Provides door-to-door transportation as required.
  
  Ensuressafetyand welfare of passengers during loading,unloading and transit.
