Complete intake and housing-related assessments for clients entering the assessment site.

Conduct diversion conversations with clients to determine whether safe and appropriate alternatives to shelter are available.

Explore potential housing options with clients, including returning to a previous residence, staying with family or friends, shared housing, or other appropriate community-based options.

Identify barriers preventing clients from accessing available housing and work with the client to develop realistic solutions.

Provide information and referrals to housing resources, rental assistance, benefits, community programs, and other services that may support a client in securing or maintaining housing.

Assist clients in identifying housing opportunities that may allow them to avoid shelter placement or move from shelter to permanent housing.

Coordinate with Housing staff, Case Managers, DHS, and community providers to pursue diversion and housing opportunities.

Obtain and review client information, identification, eligibility documentation, and other required paperwork.

Explain the intake, assessment, diversion, shelter placement, and housing process to clients in a clear and respectful manner.

Enter and maintain accurate client information in CARES and other required systems.

Review client housing history, household composition, current housing situation, and barriers to permanent housing.

Identify appropriate shelter and housing options based on the client's needs and eligibility.

Assist with shelter placement and transfers in accordance with DHS policies and site procedures when diversion or other housing options are not available or appropriate.

Follow up on pending documentation, assessments, referrals, diversion plans, and housing opportunities.

Assist clients with obtaining documents needed for housing placement, including identification and other eligibility documents.

Maintain accurate records of all client contacts, assessments, diversion efforts, referrals, and placement activities.

Monitor clients awaiting placement or housing opportunities and communicate updates or concerns to the Director of Housing and Intake.

Ensure all required intake, diversion, and housing documentation is completed timely and accurately.

Provide services using a trauma-informed, client-centered, and respectful approach.

Perform other duties as assigned by the Director of Housing and Intake.

Associate degree in Human Services, Psychology, Housing, Public Administration, or a related field preferred.

Some college with relevant shelter, housing, intake, or social services experience may be considered.

Minimum of two years of experience working in shelter, housing, intake, assessment, homelessness services, or a related social service setting preferred.

Experience working with individuals experiencing homelessness or housing instability preferred.

Knowledge of NYC shelter, housing, and social service systems preferred.

Experience with CARES or other client management and case management systems such as CAPS, HOMES, and CURRENT preferred.

Knowledge of housing resources, diversion services, benefits, rental assistance, and permanent housing programs preferred.

Strong intake, interviewing, problem-solving, and organizational skills.

Ability to communicate effectively and respectfully with clients from diverse backgrounds.

Ability to work collaboratively with DHS, housing providers, social service staff, and community-based organizations.

Strong written and verbal communication skills.

Ability to maintain accurate and timely documentation and protect confidential client information.

Ability to work effectively in a fast-paced assessment site and manage multiple client needs simultaneously.

Must demonstrate sound judgment, professionalism, patience, and the ability to maintain appropriate boundaries.

Bilingual skills are a plus.
