KAVI is a nonprofit 501(c)(3) youth-serving organization with a mission to prevent and eliminate interpersonal violence from the lives of young people through advocacy, peer leadership, community mobilization, and social justice.
KAVI promotes community wellness and healing by operating high-impact youth violence prevention and intervention programs in Brooklyn. Our programs take place in hospital-based, school, and community settings. We are a fast-paced, rapidly growing nonprofit organization. KAVI works at Kings County Hospital: KAVI runs a violence intervention program to support youth who are both perpetrators and victims of violence to access the tools they need to be safe, healthy, and thrive. Our program helps young people be better able to cope with their trauma, address and de-escalate conflict, and become active young leaders.
KAVI works in the community:
We create community engagement programs that bring family and neighbors together to discuss and act on violence prevention. Our programs help community members to advocate for our youth, understand and practice restorative justice, and participate in collective healing.
To provide ongoing case management for victims enrolled in the Program through comprehensive assessment, planning, implementation, and overall evaluation of individual needs. The overall function of the position is to act as a liaison between the client and available services with the goal of promoting continuity of care and meeting program goals. The Intervention Specialist role integrates the functions of case management and advocacy for participants and their families.
Has accountability for the enrollment and coordination of services for participants in care.
- Enrollment and Assessment of each client.
- Complete expanded assessment of clients and family needs at the time of admission.
- Maintain documentation of all activities, qualitative data, and services provided for all program participants.
- Consult Program Coordinator before forming partnerships.
- Provide every victim with an initial KAVI care package.
- Communicate with the Program Coordinator and staff about client participation weekly.
- Provide monthly reports on participation, services, and outcomes.
- Maintains a referral database and system for the referral and coordination of wraparound services.
- Acts as an advocate, investigates and reports adverse occurrences, and performs intervention as necessary
- Promotes effective and efficient utilization of community resources.
- Mobilizes resources and interviews participants, as needed, to achieve the expected goal to assist in achieving desired program outcomes within the desired timeframe.
- Conducts review for appropriate utilization of services from admission through discharge from the program.
- Conducts quarterly case management updates; assesses participants' progress through a case plan.
- Educates participant and family explaining the Intervention Specialist role and program process
- Establishes a collaborative relationship with participant and/or family to maximize the participant's and/or family's ability to make informed decisions.
- Provide program, process, and service education as identified through assessment.
- Participate in community events as appropriate by the Program Coordinator.
- May be called upon to represent program to other agencies, community-based organizations, or potential partners (Speaking Engagements)
- Coordinate crisis counseling for victims of intentional, violent injuries who are seen in the Emergency Room Department.
- Other duties as assigned.
A Bachelor's degree in a specialized field of work such as social work, healthcare, or criminal justice. Must have a minimum of one year experience in the delivery of human services and/or case management, specific to the needs and abilities of the identified program population. The successful candidate must have the physical ability to walk long distances and climb stairs.
Work Schedule/LocationHours: 12pm-8pm
Location: Central Brooklyn, including Kings County Hospital and surrounding areas.
Compensation: $54,000/yr
