Director of Social Work

Job Summary

Executive Search Network/Platinum Staffing is seeking a highly skilled and experienced Director of Social Work to join our team at a skilled nursing, long-term care facility located in Suffolk County, New York
. As a key member of our team, you will be responsible for providing exceptional social services to our residents, as well as leading and managing a team of social workers.

Job Responsibilities

• Assist new residents and their families in the initial adjustment phase, as well as in the ongoing transition to institutional living

• Provide information to residents and/or family members on residents' rights and responsibilities

• Interview residents/families to obtain social history, and participate in discharge planning, development and implementation of social care plans and resident assessments

• Interpret social, psychological and emotional needs of the resident care team members, and involve the resident/facility in planning objectives and goals for the resident

• Refer residents/families to appropriate social services agencies as needed and arrange transportation to other facilities when necessary

Qualifications/Requirements

• Must have experience as a Social Worker in a Nursing Home

• Strong written and verbal skills

• Excellent communication skills

Requirements

• Bachelor's degree in Social Work or related field

• Minimum 2 years of experience as a Social Worker in a Nursing Home

• Strong leadership and management skills
