Position OverviewThe Older Adult Services Coordinator will serve as LSA’s leader on the needs, strengths, and resources of adults age 60 and older, building LSA’s capacity to better serve and engage older adults across all of our programs. This position combines direct case management with agency-wide capacity building, staff training and consultation, community outreach, partnership development, and participant-driven program design. The Coordinator will work alongside LSA’s program teams to address older adults’ needs within our existing services, identify gaps and opportunities, strengthen connections to aging-service resources, and help shape LSA’s long-term strategy for supporting healthy aging in East Harlem.
ATypical Day
You begin your day with a home visit to assess an older adult’s housing, benefits, and healthcare needs. Back in your office, you document the case in LSA’s database, consult with the Family Support team, and follow up with a housing assistance program. Later, you lead a focus group, plan a wellness activity based on participant feedback, and help to train frontline staff on benefits and resources for older adults.
You balance responsive help for individual clients with the longer-term work of building coordinated, welcoming services that reflect older adults’ needs, interests, strengths, and leadership.
- Serve as LSA’s internal resource on older adults’ needs, strengths, benefits, services, and community resources, providing guidance and consultation to staff across programs
- Work with program directors and frontline staff to integrate practices and services responsive to older adults into LSA’s existing programs and identify opportunities for new programming
- Develop and deliver training, professional development, case consultation, and practical tools to help staff understand older adults’ needs and strengths, navigate available benefits and resources, provide culturally responsive services, and recognize age-related bias
- Provide person-centered case management for older adults with complex needs, including assessment and coordination of benefits, healthcare, mental health, housing, food, home safety, and other supports
- Conduct home visits, community outreach, service planning, follow-up, and timely case documentation
- Lead surveys, focus groups, listening sessions, and other needs-assessment activities and use findings to inform LSA’s older-adult strategy and programming
- Develop participant-driven, culturally responsive programming promoting health, nutrition, wellness, social connection, intergenerational engagement, volunteerism, leadership, and advocacy
- Create and maintain an older-adult services manual and comprehensive resource and referral guide for staff and volunteers
- Develop and maintain strategic relationships with aging services providers, healthcare organizations, housing organizations, and other community partners for referrals, learning, and collaboration
- Convene and coordinate staff, community partners, and older adults to strengthen LSA’s approach to healthy aging and identify opportunities for collaboration across programs
