EAC Network in Long Island, NY, is seeking a full-time Case Manager for the Suffolk Substance Use Disorder Transitional Case Management Program. Provide planning and care coordination for adults with SUD exiting local correctional facilities and returning to Suffolk County.
Responsibilities include meeting with jail staff, developing service plans, connecting to housing, benefits, and aftercare, plus Medicaid activation and transportation as needed. Valid NY driver’s license required.
