Bronx Family Network is seeking a full-time Case Manager to support residents in a DHS shelter in NYC. You will conduct needs assessments, develop ILPs, provide case management, crisis intervention, and connect clients with housing, health, and social services.
You will document in CARES, maintain accurate records, advocate for clients, and work closely with shelter staff to move clients toward permanent housing. Bilingual English/Spanish preferred;
Microsoft Office proficiency required.
