Position OverviewThe Columbia County Homeless Intervention Case Manager(“CCHI”) assists families and individuals with all facets of the intake process. This includes performing assessments, service plan development and coordinating housing activity with families. The Case Manager will also coordinate service provisions with the Family Care Coordination Program .
Position Qualifications & Requirements- High School Diploma / GED equivalent required;
Experience in human services preferred; - Associate/Bachelor’s Degree with limited experience.
- Professional record of integrity and strong advocacy for homeless families.
- Ability to work flexible hours to accommodate client schedules, which may include evening and/or weekend hours, including rotation of holidays.
- Local travel is required; A current Clean and Valid New York State Driver’s License (actively licensed at least one year) - is required *.
- Knowledge of community resources is beneficial.
- Previous experience working in a human services agency is a plus!
- Proficiency in computer applications:
Microsoft Office & Google Business Workspace Suites; and secure Internet practices. - Outstanding interpersonal and communication skills;
Capable of building strong working relationships with regulatory bodies. - Self-motivated and engaged, with a strong attention to detail;
Excellent writing aptitude. - A compassionate, warm and caring mindset;
Capable of exhibiting empathy for those in crisis. - Capacity and openness to work with populations from culturally diverse and economically disadvantaged backgrounds, including but not limited to a broad range of people with various developmental and functional levels.
