Bronx Works is seeking a case management professional to support families across Bronx programs including health, housing and social services. You will assist with intake, service planning, referrals, advocacy and follow-up, managing a caseload and documenting progress.
The role requires a relevant degree or experience, familiarity with HRA systems, and strong communication and organizational skills. Travel is possible within NYC boroughs for home visits and field work.
