Bed seeks a dedicated caseworker to support consumers living in independent apartments in the community. Under supervision, you help individuals develop living, working and social skills and connect them with medical, social and educational services.
You will travel to residences, maintain case records, implement service plans, provide crisis intervention and coordinate with a multidisciplinary team to promote recovery and independence.
